Medication Management Process
- Medication Ordering and Acquisition: The facility may handle refilling prescriptions or coordinate with residents’ pharmacies. Family members may also be involved in this process.
- Medication Review: A healthcare professional (doctor, nurse, or pharmacist) will review a resident’s medications to ensure they don’t interact poorly or cause adverse effects.
- Individualized Medication Plan: A plan is created outlining the specific medications a resident needs, dosage amounts, and frequency of administration.
- Medication Administration: Trained staff (nurses, aides) may dispense and administer medications orally, topically, or through injections (depending on the resident’s needs and the facility’s policies).
- Monitoring and Communication: Staff will monitor residents for any side effects or reactions to medications and report any concerns to healthcare professionals. Residents and their families will be informed about any medication changes.
